ABSTRACT

Hemophilia is a congenital disease resulting in deficiency of clotting factors. For this reason they have a constant need of clotting factors which makes them one of the largest consumers of blood products. Through this blood product use, many of these patients have become infected by hepatitis C virus [HCV]. We have studied the seroprevalence of anti-HCV antibody and HCV infection in hemophilic patients refereeing to Iran Hemophilia Society center in Tehran. In a cross-sectional study we reviewed files of all hemophilic patients registered in Iran Hemophilia Society center in Tehran from 2003 to 2005. Subjects with available results of anti-HCV antibody were included. Among the 1170 files examined, 1095 contained data on HCV infection status. From these, 802 subjects [72.3%] had anti-HCV antibodies. Genotype 1 was most common [60%] followed by genotype 3 [38%]. Almost 10% of patients with anti-HCV antibodies had a negative HCV RNA by PCR. HCV is very common among hemophilic patients and considering the risk for the patients and their contacts it is necessary to screen all hemophiliacs for HCV and treat if indicated

ABSTRACT

The thyroid gland is subjected to a series of investigation of whom the [cytopunction] which allows for a high reliable differentiation between the benign thyroid nodules, the suspicious nodules and those prankly malignant. This study aimed for the evaluation of the cytological results and to observe the pertinence of these by confronting then with the histological study. It is about a retrospective study concerning 1090 thyroid cytopunctions practised in the pathological anatomy department in hospital Ibn Sina of Rabat through period of 11 years [2000-2010]. The benign cytological results was represented by 1019 against 1090 that is 93,8 %. The suspected cytology was 57 among which 30 were analyzed histologicaly. In 57 atypical cases 9 were frankly malignant that is 16,5 %. This percentage is not insignificant and incites to operate all the suspected cases. The malignant cases in the cytological were 14 among which 11 were confirmed histologicaly that is a 81,5 % of sensibility. The comparative study of the series of [1989 - 1999] and that of [2000-2010] showed a clear progress at every level. The average survival rate was the same for both series. It is of 45 years, with a feminine prodominancecy. The cytological thyroidal studies are the most objective and the most significant during these last years. it showed its relevance and its efficiency by confronting all the atypical or suspicious cytology with that of the histological examination. At present the standard diagnosis can benefit from the immunohistochemistry

ABSTRACT

It is a very rare affection even in our country which is a tuberculous endemic country. The physipathological mechanism is still controversial, lithiasis plays a role in supporting the tuberculous localization in the gallbladder which decreases the vesicular resistance against bacillus of Koch. It is about a case of tuberculosis of the gallbladder of an operated patient for chronic lithiasis cholecystitis which appears to have a direct relation with the vesicular lithiasis. Its diagnosis depends on the histological examination of the piece of cholecystoctomy. This pathology represents 1% of the digestive turberculosis localization. Its clinical signs are not specific. The biological assessment shown the presence of blooc lymphocytosis which is in favor of a possible tuberculosis, but rarely found. The only certitude diagnosis is the anatomopathological examination of the excised piece of cholecystoctoy by finding the evidence of the epitheloidal and granulomatous inflammation and cellular overgrowth with caseous necrosis. The physiopathological mechanism is still discussed. The presence of calculus resulting in disappearance of biliary acids and provokes a non specific inflammatory reaction to the vesicular mucosa which decreases the vesicular resistance against bacillus of Koch. This possibility shows an increase in frequency in the developed countries due to the emerging of AIDS. The impact of the lithiasis on the appearance of this specific inflammation is possible but remains discussed. Its treatment is medico-surgical

ABSTRACT

Characterization of Leishmania parasites is necessary for epidemiological objectives such as documenting the distribution of predominant species and designing appropriate control measures. In this study, we aimed to identify Leishmania species isolated from cutaneous leishmaniasis [CL] patients, using RAPD-PCR method. This descriptive, cross-sectional study was designed against 20 Leishmania spp. which were confirmed by parasitological examination, isolated from 30 suspected cutaneous leishmaniasis patients, referred to Health Centers of Kermanshah Province from August 2006 to December 2007. All desirable samples were characterized by RAPD-PCR method using five selected oligoprimers [AB1-O7, A4, 327, 329 and M13]. Electrophoresis patterns from each isolate were compared with reference strains of L. tropica, L. major and L. infantum. Eighty nine percent and 11% of isolates were similar to L. major and L. tropica reference strain, respectively. Five of the isolates were identified as L. major using RAPD-PCR, induce ulcers at the base tail of Balb/c mice, 4-6 months after inoculation. L. major is dominant in the studied areas and it seems that some parts of the Kermanshah Province to be probably considered as zoonotic cutaneous leishmaniasis areas in the middle west of Iran

ABSTRACT

Solitary fibrous tumors are mescenchymal neoplasms which develop from connective tissue. They rarely occur in the kidney and to our knowledge only 11 cases have been reported in the literature so far. We herein report a solitary fibrous tumor of the Kidney in a 60-year-old patient presenting with renal colic. CT scan revealed a tumor in the renal hilum. The patient was subjected to total nephrectomy, and histological examination of the excised tissue showed fusiform cellular fibrotic proliferation. Immunohistochemistry confirmed the diagnosis by showing positive CD 34 and bcl-2 markers

ABSTRACT

Toluene is an organic solvent that it is one of the byproducts in the coke industry. Exposure to toluene causes central nervous system dysfunction and others disorders. Many workers are exposed to toluene due to leakage from tracks. Therefore the aim of this study was to determine the levels of exposure through environmental and biological monitoring of toluene Air toluene sampling of air inhaled by 36 coke oven workers was done by using activated charcoal tubes and personal sampling pumps. At the e[1id of the shift, urine samples of workers and control group were collected by urine samplers. Air toluene was determined by GC, urinary hippuric acid by FIPLC and urine creatinine level was determined by auto analyzer. The mean of air toluene and urinary hippuric acid levels in exposed and control samples were 14.34 ppm, 0.33 and 0.28 g/g creatinine. Air toluene and urinary hippuric acid showed a correlation of r = 0.8. Toluene TWA was lower than the TLV-TWA [p=0.000]. Urinary hippuric acid concentration was also lower than the BEI [p=0.008]. Difference between exposed and unexposed group was not significant. This study showed that hippuric acid because of its interaction with background factors can not be used as a sensitive biomarker for biomonitoring

ABSTRACT

Although BCG vaccination has many benefits, it can cause multiple complications. Disseminated TB is the most severe but rare complication of BCG vaccination. Osteomyelitis is another important complication. Usually these complications are due to a defect in immune system. Rapid diagnosis and treatment can be life saving. The first case was a 3 month old boy that was admitted in hospital because of fever, lethargy and fistula in the right axillary area at the site of BCG vaccination. The second one was a 7 month old boy with an ulcer and a fistula at the site of BCG vaccination and chest wall. Diagnosis of disseminated TB was made by biopsy and culture. All immunologic investigations, except interleukin 12-gamma interferon pathway, were normal. When a child presents with lymphadenopathy and ulcer near the site of BCG vaccination, the physician must perform a complete physical examination to exclude diagnosis of disseminated TB, especially in the presence of long term low grade fever, organomegally, skin lesions and lack of weight gain

ABSTRACT

The idiopathic granulomatous mastitis is a rare entity. Up till now there is no any theory that can explain this pathology, specially the auto-immune origin. So we can consider it as of unknown aetiology. It is often adopt a cancerous radio-clinical semiology, while the histological examination proves that it is of a benign nature. Its diagnosis depends on a multidisciplinary approach. It is of an exclusion diagnosis, which needs an adequate treatment, with a long course follow up. We report here a new idiopathic granulomatous mastitis case, through a review of literature, trying to present its epidemiologic and clinical aspects, discussing the diagnostical methods and the therapeutic difficulties

ABSTRACT

The cancer of the kidney accounts for 3% of all adult malignant tumors and have an increased incidence. This retrospective study, reports 139 cases colliged in the Ibn Sina hospital, between 1990 and 1999. It concerns 68 men, 72 women [range 19-88 years]. Most of patients [79.8%] have between 40 and 70 years. The diagnosis was assessed by the histological examination about surgical specimens in 132 cases, renal biopsy in six cases and renal cytopunction in one case. Clear cell carcinoma was more frequent [72%] in 100 cases, metastasis [20.3%] in 28 cases, tubulo-papillary tumors, sarcomatoide carcinoma and wilms tumor [2,1% each other] 3 cases, Bellini carcinoma and clear sarcoma [0,7% each other] one case. Furhman grade as a prognosis factor used in 103 cases that 65% had a high grade [III, IV]. The diagnosis is based by microscopic gross in all of case except in few cases, the immunohistochemistry study was necessary. We insist on the central role of pathologist in prognosis of cancer kidney in adults

ABSTRACT

Kaposi's sarcoma was described in 1872. [Kaposi]. It affects particulary the skin, the mucous membrances, the intestines and the ganglions. In addition to the sporadic "classical" type, there are three associated types: the endemic "african" type the "allograft associated" type and the epidemic type associated with the acquired immunodeficiency syndrome [A.I.D.S.p.]. These four types are different but they have the same histological picture Kaposi's sarcoma may be due to immunological factors or viral or genetic causes. The therapy is different according to Kaposi's sarcoma type: we use radiotherapy, chemotherapy, interferon and AZT in AIDS'S form

The pathogenesis of amyloidosis remains unclear. The ultrastructural and biochemical studies show that the different types of amyloidosis are generally correlated with the primitive or secondary pattern of the disease. Pathological study, using different special staining, especially the Wright technique, confirms the diagnosis of amyloidosis and may establish its primitive or secondary pattern. ABSTRACT

Ganglioneuroma and neuroblastoma are both tumors of the sympathetic nervous system. Neuroblastoma is a malignant tumor, but ganglioneuroma is a benign and a mature one. The study deals with an uncommun aspect of the ganglioneuroma which is revealed by multiple cutaneous metastasis. Examination revealed two others mediastinal and retroperitoneal tumors which are asymtomatic. We think, as the majority of investigators that the tumor was previously a neuroblastoma that metastased at the skin and matured of ganglioneuroma with primary and metastatic tumor. Surgical treatment of both mediastinal and retroperitoneal tumors had led by itself to a regression of subcutaneous with the tumors mentioned above
